Church issued licenses cannot be accepted.
Marriage license must be from a governmental agency. In some instances, more than one marriage license, divorce decree or court order document may be required to show the chain of name changes. If you have recently changed your name, update your records with the Social Security Administration before you apply for your Florida Driver License. The name assigned to the social security number must match the name that will appear on your Florida Driver License. Hospital birth certificates cannot be accepted. A birth certificate must be issued by a government agency.
